Application for Service

Complete the Application for Water Service form and e-mail it along with the required documents to WaterServiceRequest@mlbfl.org. You will receive a response within two working days.

OR

Bring completed application along with the required documents to the Utility Counter located on the first floor at City Hall, 900 E. Strawbridge Avenue, Monday – Friday, 8:30 a.m. – 4:30 p.m.

Required Information/Documentation

To establish a new utility account, please provide the following:

  • Customer's Full Name
  • Service Address
  • Driver's License or ID
  • Documentation of Lease (rental) or Ownership (proof of purchase)

Deposit

The deposit amount varies and may be billed on the first bill for customers without outstanding balances. A $20.00 service initiation fee will also appear on the first bill.  Past due bills are subject to disconnection.
