Street light pole identification number Street light pole identification number Street light poles have an eleven digit identification number affixed to them, about three feet above the ground. This identification number will begin with numeral four, such as 4-8511-2111-11. Please record the number listed on the pole, or if the pole has no number or the number tag is damaged, please write down the exact address.

This address must include which street the pole is located on and the house or building number closest to the non-working light on the same side of the street.
